Issue: Missing City/State Fields on Client Mentoring Request Form
System/Platform: Client Intake Form 641 (Engage)
Description:
When a client submits a mentoring request using the client intake form, the City and State fields are not displaying in the submission details, all other fields including the zip code are showing at this time.
Current Status:
The development team has been notified. However, there is no estimated time of resolution (ETA) at this moment.
Temporary Workaround:
- Try using the ZIP code manually to determine city/state: https://tools.usps.com/zip-code-lookup.htm?citybyzipcode
- Additional follow-up with the client may be required in some cases
