SCORE Minor Client Consent Form

Any participant of a SCORE workshop, mentoring program or other SCORE activity that is under the age of eighteen (18) must have their legal parent or legal guardian consent to the individual’s participation in the event. SCORE does not permit anyone under the age of 13 to participate is SCORE activities.

Please have the parent or legal guardian complete and sign the below linked form and return it to the SCORE mentor or client intake coordinator.

SCORE staff or volunteers collecting this completed form must attach it to the electronic client record in Engage.
