SCORE provides a set of essential Financial and Compliance Documents to support transparency, accountability, and fundraising activities. These resources help chapters and volunteers manage donor acknowledgements, access IRS filings, and use approved templates and forms when working with partners or donors. Having these materials in one place ensures SCORE maintains consistency, compliance, and credibility across the organization.
Available Tools
Field Sponsorship Agreement
Chapters should use this agreement whenever securing cash contributions, in-kind donations, or event sponsorships from local partners. This should be used when there are specific benefits or deliverables for the sponsor. For example, logo recognition, event visibility, or named support for a program. It documents the sponsorship amount and outlines what SCORE will provide to the sponsor in return.
SCORE Invoice Template
The Invoice Template should be used when a sponsor or partner has made a verbal or written commitment to provide support, and they need an invoice in order to process the donations through their internal system. It serves as the formal request for payment when a sponsor's finance team requires an invoice. For example, when a company pledges a sponsorship during a meeting, and asks for an invoice to submit.
Form 15 – Acknowledgement of Gift to SCORE
This form is used to acknowledge and document donations made to SCORE in compliance with IRS requirements for charitable contributions. It is used after a contribution has been received as the official donation acknowledgement for tax purposes, confirming the gift amount and SCORE's non-profit status. These are automatically generated by Accounting. Please note, acknowledgment in other forms is encouraged as this is a tax document rather than a "thank you".
IRS Affirmation Letter
Find the official IRS affirmation letter confirming the tax-exempt status of the SCORE Association. Please note this is equivalent to an IRS Determination Letter.
W-9 Form
Access W-9 forms for SCORE Association and chapters to provide to donors, partners, and funders as needed.
